Sorry not clear.

Push is instant message delivery, the opposite of "only if I refresh".

Do you want the app to not check mail at all, unless you refresh?

Turn off app settings -> mail, receiving -> scheduled sync *and* IMAP push

Is scheduled mail checking also enabled?

Is account-level push enabled? It's off by default.

Long press the account -> options and folders -> account options -> enable IMAP push.

These are my Gmail account settings:
[which are working fine - push notifications immediately]

1. Screenshot:
> long press on Account name > Options and folders > Account options

2. Screenshot:
> long press on Account name > Options and folders > IMAP options

3. Screenshot:
long press on Account name > Options and folders > Filders > tap on folders name "Inbox"

4. Screenshot:
> tap on folders settings wheel (on the right edge)
